How does topical testosterone work?

Topical testosterone is absorbed through the skin into the lymphatic system — a transdermal route that bypasses first-pass liver metabolism. Once-daily application keeps your blood levels steady through the day, instead of the peak-and-trough swings you get with weekly injections.

How do I apply it?

Apply the prescribed dose once a day to thin-skinned areas — inner arms, wrists, lateral abdomen, inner thighs, or scrotum — and avoid fattier sites where absorption is unreliable. Let the gel dry fully before dressing, and wash your hands. Dr. Salim’s protocol gives you the exact site rotation, dose, and timing.

When will I notice changes?

Topical testosterone absorbs quickly — blood levels peak within hours of application. Most men notice subtle shifts in energy, mood, and libido inside the first few weeks. The fuller effect builds gradually as levels stabilize, typically over two to three months of consistent daily use.

What are the potential side effects?

The most common are skin reactions at the application site (redness, irritation, oily skin), acne, and shifts in libido or mood. Topical testosterone can also raise hematocrit and hemoglobin, and can move PSA — which is why ongoing bloodwork is part of the protocol, not an afterthought. The one most men ask about is hair loss: testosterone converts to DHT, and in men with a male-pattern predisposition, more DHT can accelerate thinning. It doesn’t cause hair loss in men who weren’t already going to lose hair, but it can speed up what was already on the cards. The hepatotoxicity risk that’s been associated with some older oral testosterone formulations isn’t seen with transdermal delivery. Liver enzymes are still tracked as part of routine bloodwork.
