The tablet must be swallowed whole with a small sip of water — no more than four ounces — at least 30 minutes before any food, drink, or other oral medication of the day. This timing window is non-negotiable for the medication to be absorbed properly; deviating materially reduces how well the medication works.
Who shouldn’t take it.
- Anyone with a personal or family history of medullary thyroid carcinoma, or with multiple endocrine neoplasia syndrome type 2.
- Anyone with current or past pancreatitis.
- Anyone with severe gastrointestinal disease, including severe gastroparesis or significant motility disorders.
- Anyone who is pregnant, planning a pregnancy, or breastfeeding.
- Anyone with severe renal impairment, until evaluated by your Gambit specialist.
- Anyone with severe liver impairment, until evaluated by your Gambit specialist.
- Anyone who has previously had a serious hypersensitivity reaction to this medication or any of its components.
Common side effects. The most frequently reported effects are nausea, diarrhea or constipation, decreased appetite, and abdominal discomfort, especially during the first month and during each dose increase. Gradual titration is designed specifically to reduce these tolerability issues, and most symptoms ease as the body adjusts to each dose level. Less commonly, some patients experience headache, fatigue, dizziness, or a temporary feeling of fullness after smaller meals. Because this medication slows the rate at which the stomach empties, it can change how other oral medications you take are absorbed, including thyroid medication; tell your Gambit specialist about every medication and supplement you take so the timing and dose can be reviewed.
When to seek medical attention.
- Severe persistent abdominal pain, especially pain that radiates to the back, which can signal pancreatitis.
- Pain in the upper-right abdomen, yellowing of the skin or eyes, persistent nausea, or fever, which can signal gallbladder issues, including gallstones.
- Signs of an allergic reaction, including rash, swelling of the face or throat, or difficulty breathing.
- Severe persistent vomiting or diarrhea, signs of dehydration, or an inability to keep fluids down.
- A lump or swelling in the neck, persistent hoarseness, difficulty swallowing, or shortness of breath, which can signal a thyroid issue.
- Symptoms of low blood sugar — shakiness, sweating, confusion, racing heartbeat — particularly if you also take insulin or other diabetes medications.
Monitoring. Comprehensive bloodwork is required before prescribing, and your Gambit specialist reviews markers including fasting glucose, HbA1c, fasting insulin, and ACTH at scheduled follow-up assessments. Clinical-status check-ins happen during the dose-escalation period, and ongoing specialist re-evaluation continues across the first several months until your protocol stabilizes. Cadence is set by your Gambit specialist based on your clinical picture, not by a fixed calendar.

Are there special instructions for taking a Needle-Free GLP-1 Pill?
Yes — and the instructions matter. Take one tablet first thing each morning, swallow it whole with no more than four ounces of water, and wait at least thirty minutes before your first food, coffee, or other medication of the day. The thirty-minute fast and the small sip of water aren’t suggestions: they’re what allows the medication to be absorbed properly. What side effects should I be aware of?

How long until I see results, and how often will I see a Gambit specialist?
